Imperfect results are not the seeds of medical malpractice

Clearly, there's no shortage of medical malpractice in the U.S. I'm not talking about lawsuits. I'm talking about actual medical malpractice, as in medical errors. We've seen the numbers over and over again, but they bear repeating because frankly, the gross numbers are staggering (even if, on a percentage basis, the number of reported incidents is actually, gulp, bordering on acceptable).
