Palomar Pomerado Health hired Janine Sarti in 2007 to institute the first in-house legal department in the San Diego public health district's 60-year history. "PPH used outside firms who did a very good job, but there was a cost - a significant cost - associated with it," Sarti said. Outside counsel fees ranged from $2 million to $6 million annually before Sarti's arrival, and now average $200,000 to $300,000 a year.
